Please fam- iliarize yourself with your insurance benefits prior to your appointment to minimize any confusion regarding your plan coverage. A co-payment or deductible may be due at the time of service. Please consult your health plan to determine these benefits. If we do not participate with your health plan, payment for services rendered will be collected in full at the time of service. As a courtesy, we may file a claim with most insurance plans. |
